One fact should organise this whole day and it usually gets organised around a gift instead.
She has not eaten or drunk anything since before sunrise. Not since dawn. Through a working day, in most households now, and frequently while also cooking for the evening.
By moonrise she is fifteen hours into a fast. That is the context the gift arrives in, and the best thing you can give is shaped by it.
What actually helps on the day
Before any object:
Take the cooking. The sargi is eaten before dawn and the meal after moonrise is usually elaborate, and in a large number of households the woman fasting is the one preparing it. Doing that, or having it done, is worth more than anything bought.
Handle the household. Children, guests, the arrangements, whoever needs collecting from where.
Do not schedule anything for her. No plans that require her to be somewhere, dressed, performing, before she has eaten.
Be home for moonrise. Being late for the one moment the entire day is built around is the single failure that is remembered.

The gift, and its scale
Karwa Chauth is conventionally a jewellery occasion in a lot of families, and where that is the convention there is nothing to argue with.

Give it after she has eaten. Not at moonrise, not during the ritual — later, when the day is finished and there are no guests. Something that asks for attention while she is fasting is badly placed however good it is.
The archive nobody uses
Karwa Chauth is annual and it photographs well: the mehendi, the thali, the sighting through the sieve, the whole household in one place.
Every year those photographs go into a phone and stay there. Printing last year’s and giving them at this year’s is a habit rather than a gift, and by year five it is a record of the marriage that nobody planned.
It also solves the recurring problem with this occasion — that the conventional gift escalates year on year and eventually has nowhere to go.
What to avoid
Anything that arrives during the fast and needs a reaction. Wait.
Escalating jewellery annually. It sets a floor that only goes one way, and by year ten the gift is an obligation with a rising minimum.
Kitchen equipment. On a day whose burden is largely kitchen work, this reads exactly as you would expect.
A surprise that requires her to go somewhere. She is fasting.
Nothing at all, if your household observes it. The one genuine misfire.
Timing

When she has said she does not want a gift. Some women fast without wanting the day commercialised around them, and overriding that is not generosity.

When your household does not observe it. Not every family does, and importing an occasion because the internet is full of it is a strange thing to do.
When the honest gift is that she does not fast alone. Plenty of husbands fast alongside now. That is a larger gesture than anything on this page and it costs nothing.
