The rule that makes this work
Match their disclosure exactly.
If they told you privately, the gift is private. If they have not mentioned it at all and you only know because you guessed, the correct response is probably a message rather than an object.
A gift is not the place to reveal that you know something. That is the single way this goes wrong, and it goes wrong badly.

What not to put on it
No dates. A date is a reference to the thing, and it makes the object unusable in front of other people.
No numbers. “One year” is the same problem.
Nothing that needs explaining to a visitor. The test is whether they could leave it out on a table without anybody asking a question they do not want to answer. If not, it is the wrong object.
When the answer is no object at all
When you are not sure they see it as an achievement. Some things people are relieved about rather than proud of, and a gift reframes it as a triumph they may not feel.
When it is ongoing. Marking a midpoint can read as pressure.
When you found out by accident. Then it is not yours to mark.
In all three cases, saying one true sentence is better than any object.
