Three ways round it:
Ask HR or an office manager. Most companies hold addresses and will send something on your behalf, especially at year end.
Ask them plainly. “I want to send you something, what is your address?” Slightly less of a surprise and considerably less faff.
Send it to whoever is nearest them who does know — a shared manager, a colleague in the same city.
